Gentamicin Dosing (Extended Interval)
Calculates the initial dose for once-daily (extended-interval) Gentamicin based on adjusted body weight and estimates the dosing interval using the Hartford nomogram principles.

*Measure level 6-14 hours after the start of the first infusion and plot on the local nomogram to confirm ongoing interval.
Clinical Principles
- Aminoglycosides exhibit concentration-dependent killing and a significant post-antibiotic effect.
- Obesity: If actual body weight is >20% above IBW, Adjusted Body Weight (IBW + 0.4(ABW - IBW)) must be used to prevent toxicity, as gentamicin does not distribute well into adipose tissue.
- Contraindications for Extended Interval: Ascites, severe burns, pregnancy, endocarditis (requires synergy dosing), CrCl < 20 mL/min.
