For pharmacists undertaking their Independent Prescribing qualification, your Designated Prescribing Practitioner (DPP) requires evidence mapped directly to the RPS Competency Framework. This guide shows how to write those clinical logs.

The RPS Competency Domains

The framework is divided into two areas: The Consultation and Prescribing Governance. Your portfolio entries should explicitly state which competency they demonstrate.

1. Assess the Patient

Common Error: Writing "I took a history."

Better: "I undertook a structured WWHAM and ICE consultation for a patient presenting with dyspepsia. I palpated the abdomen to rule out acute red flags (e.g., guarding) and documented the absence of alarm symptoms (dysphagia, unexplained weight loss) as per NICE CG184."

2. Identify Evidence-Based Treatment Options

Common Error: "I decided to prescribe Amoxicillin."

Better: "I diagnosed acute otitis media. Referencing the local formulary and NICE NG91, and noting the patient's penicillin allergy (anaphylaxis), I opted for Clarithromycin 500mg BD for 5 days. I calculated the dose based on the patient's renal function (CrCl > 60ml/min)."

3. Present Options and Reach a Shared Decision

Common Error: "I told the patient to take the tablets."

Better: "I explained the risks and benefits of antibiotic therapy vs delayed prescribing. I utilised a 'safety netting' approach, advising the patient on specific worsening symptoms (e.g., mastoid swelling, systemic fever) that would necessitate immediate A&E attendance. The patient opted for the immediate script, agreeing to the management plan."
